The ​SWMS Counseling Department is open from 8 a.m. until 2:30 p.m. and can be reached at the school by calling 912-395-3540 (option #2), or you may email your scholar's grade level counselor at the addresses below.

Student Appointment Request Form

Students may fill out the following form to request an appointment with a school counselor. Once the form is filled out, a counselor will call the student for an appointment. This form is for students to request a non-urgent appointment with a school counselor. If you need to report a safety concern for yourself or a friend, DO NOT fill out this form but immediately speak with a trusted adult and let them know you need to speak with someone immediately.
